This week during clinical rotation I have seen a couple different patients with ear infections, one strep throat, and one with pink eye. I have only experienced one clinical day to reflect on this topic, so I am limited on what I have seen. The first patient that I saw was 12 years old and came in with complaints of sore throat and fever lasting three days. Mother states initially the patient did not have white patches on his throat, but he has since developed them since the evening prior. Patient states he feels like he is swallowing razors because the pain is so bad. Patient was not able to eat and drinking chicken noodle soup was too painful and burned so he has been drinking water and Gatorade at times to remain hydrated. Patient had sore throat, swollen tonsils that were red and covered with white patches, and fever of 101.5F in office all which present with strep/acute pharyngitis. Patient was prescribed Penicillin 500mg PO TID x 10 days (Seattle Children’s Hospital, 2022).
Another patient was 4 years old, and father presented her to office. Father states that she has been whiny and not herself for a few days but started running a fever last night and tugging at her right ear. He states patient usually gets a couple ear infections a year, but she has not experienced one this year. After observing patient’s ears, patient noted to have otitis media in bilateral years with the right being worse than the left. The right ear with more swelling and redness noted. Father states patient has had a cold and runny nose for the last week but appeared to be getting better than spike a fever last night of 102.2F. Patient in office with fever of 101.9F. Based on patient’s weight, patient prescribed Amoxicillin 200mg/4ml PO BID x 10 days. Educated father to bring patient back if symptoms continue (Johns Hopkins Medicine, 2022).
A patient arrived 8 years of age with red, itchy, and draining left eye which are all symptoms of conjunctivitis (pink eye). Mother states that the child is attending a summer program where there have been a few other kids with pink eye. She states the patient’s eye was matted shut upon awakening this morning and she applied a warm, wet wash cloth that help get it open. When asked the patient states it feels like something is in the eye, but nothing has been observed. After observing and assessing the eye, the eye is red and inflamed with drainage noted. The patient was prescribed Azithromycin 1% drops-apply 1 drop to affected eye BID x 2 days and then 1 time a daily x 5 days. Educated mother to do warm compresses as needed and to ensure hand hygiene is done frequently to reduce the chance of spreading to others (Mayo Clinic, 2020).
