Subjective: During the psychiatric assessment, the patient provided detailed information regarding their chief complaint and symptoms. They reported having trouble focusing, difficulty sleeping, low energy levels, and an overall feeling of being overwhelmed. The duration of these symptoms has been approximately two weeks and they are having a significant impact on their functioning in life as it is making it hard for them to concentrate on tasks or even complete simple daily activities.

Objective: During the psychiatric assessment, I observed that the patient had a flat affect with little to no changes in facial expression. They spoke slowly with long pauses in between sentences which was indicative of lack of motivation or apathy. Additionally, they appeared disheveled with disorganized thought processes which could be evidence of depression or anxiety disorder. Physical exam findings were within normal limits except for mild tachycardia which may indicate distress or underlying medical condition. Overall it seemed as though there were signs of depression but further testing would need to be done in order to come up with a differential diagnosis.
