I chose to assess a semi-direct nursing situation that is in need of change – the discharge process at my local hospital. Upon observation, I found that the current discharge process is inefficient and often leads to safety issues for patients due to lack of clear communication between nurses and doctors. To address this problem, I chose John Kotter’s 8-Step Change Theory as my chosen change theory because it provides a comprehensive framework for making changes in organizations.

My proposed plan for implementing this change involves an eight step process: creating urgency, forming a powerful guiding coalition, creating visions and strategies, communicating the vision throughout the organization, empowering employees for action, generating short term wins, consolidating gains and institutionalizing new approaches.  The first step would be to create urgency by identifying why this change needs to be made and how it will benefit the organization. The second step would involve forming a powerful guiding coalition by recruiting members from various departments within the hospital who are willing and able to help implement this change. This team should include representatives from both medical staff such as doctors and nurses as well as administrative staff such as managers or quality assurance officers.
