The evaluation plan for my evidence-based practice project will be comprehensive, concise and aimed at determining the effectiveness of my intervention. To do this, I will use both quantitative and qualitative methods to collect outcome data so as to understand the extent to which the objectives of my project are achieved.

Rationale: The rationale behind using both quantitative and qualitative methods is that they provide a more thorough understanding of the outcomes by capturing different types of information (i.e., numerical data versus subjective experiences) and also allow me to evaluate how well my intervention has been implemented in terms of fidelity (e.g., adherence to protocols).

Outcome Measures: In order to measure outcomes, I will use pre/post surveys and interviews with participants, as well as performance measures such as number of participants who completed the project or attained desired goals. These measures will help me assess if there are any changes in participant behaviors or attitudes related to their engagement in the project—for example, improved knowledge about health topics being addressed or increased motivation for healthy lifestyle habits among those who may have previously been disengaged.

Data Evaluation: To ensure that outcomes are accurately measured and evaluated based on available evidence, all collected data should be reliable (consistent over time), valid (accurately represents intended meaning), and applicable (relevant enough for analysis). Validity can be established through triangulation—using multiple sources like observation logs, feedback forms etc.—while reliability can be checked through an internal audit or external expert review process.

Strategies if Outcomes Are Negative: If results from our assessment indicate that improvements were not made relative to our objectives then we need strategies in place for improvement actions including modifying interventions if necessary; re-evaluating target population characteristics; finding better ways to engage stakeholders; increasing resources allocated towards program implementation etcetera.. Furthermore further research into potential outside factors influencing efficacy should also conducted alongside creating action plans rectify underlying issues contributing negative outcomes..
